REGISTERING A STUDENT for Thomas C. Passios Elementary Registration Packets available in the Main Office. Please have the following documents available when registering your child for school.

1. Photo ID

2. Your child's original birth certificate

3. Health Insurance information

4. Copy of latest Transcripts/Report Card and Discipline Records (if applicable)

5. Copy of current Individual Educational Program or 504 Plan (if applicable)

6. Individual Health Care Plan (if applicable)

7. Legal Custody Form (if applicable)

8. Copy of School Health Record, Copy of most Recent Physical, and Immunizations.

9. Copy of child's Massachusetts Transfer Card

10. Proof of Residency - three sources (ie; purchase and sales agreement, electric bill, license, etc.)

It is not necessary for parents or guardians to bring their children with them at the time of the registration.
